Tooth Decay

When decay reaches the inner dentin layer of a tooth, the nerve becomes exposed to temperature, pressure, and bacteria. The result is sensitivity that gradually deepens into constant pain. Early decay is treated with a filling; deeper decay may require a root canal.

Dental Abscess

An abscess is a pocket of infection, either at the root tip or in the gum tissue alongside the root. The pain tends to be throbbing, severe, and constant. You may notice swelling in the jaw or cheek, a bad taste in the mouth, or a pimple-like bump on the gum. This is a dental emergency — left untreated, the infection can spread beyond the tooth.

Cracked or Fractured Tooth

A cracked tooth often causes sharp pain when biting or releasing pressure, and sensitivity to hot and cold that lingers. The crack can be invisible on an X-ray, making it one of the harder causes to diagnose. If the crack extends into the pulp, root canal therapy is necessary to save the tooth.

Gum Infection

Advanced gum disease causes the bone and tissue supporting a tooth to break down, creating deep pockets where bacteria thrive. The resulting infection produces a persistent, dull ache around the affected tooth along with bleeding and swollen gums.

Exposed Root

Gum recession from aggressive brushing, grinding, or periodontal disease leaves the root surface unprotected. Root surfaces lack enamel, so even mild temperature changes or sweet foods trigger sharp, brief pain.

Sinus Pressure

The roots of the upper back molars sit very close to the sinus cavities. During a sinus infection or allergy flare, pressure in those cavities can mimic toothache pain across multiple upper teeth simultaneously. If several upper teeth ache at once, sinuses may be the source rather than the teeth themselves.

What helps

• Ibuprofen taken as directed on the package — it reduces inflammation, which is often a major driver of tooth pain
• Salt water rinse: half a teaspoon of table salt in eight ounces of warm water, rinsed gently for 30 seconds
• Clove oil applied with a cotton ball held gently against the sore area — the active compound eugenol is a natural topical anaesthetic
• Cold compress applied to the outside of your cheek for 15 minutes on and 15 minutes off to reduce swelling
• Keeping your head elevated, especially when sleeping, to reduce throbbing pressure

What to avoid

• Do not place an aspirin tablet directly on your gum or tooth — aspirin is an acid and direct contact with soft tissue causes a chemical burn
• Do not apply heat to the outside of your face if you suspect an infection — heat can accelerate the spread of infection
• Do not ignore the pain hoping it will resolve on its own — a dental abscess does not clear without treatment

Dental Filling

If decay is the cause and the nerve is not yet involved, removing the decay and placing a tooth-coloured composite filling resolves the pain. This is completed in a single appointment.

Root Canal Therapy

When decay or a crack has reached the nerve, or when an abscess has developed at the root, root canal therapy removes infected pulp tissue, disinfects the canals, and seals the tooth. Contrary to its reputation, a properly anaesthetized root canal is no more uncomfortable than a filling. A crown is placed afterward to protect the treated tooth.

Antibiotics

If an abscess is present and the infection has spread beyond the tooth, antibiotics are prescribed alongside dental treatment. Antibiotics alone do not eliminate a dental abscess — the source of infection must be addressed directly with root canal therapy or extraction.

Tooth Extraction

When a tooth cannot be saved, extraction provides immediate pain relief. Dr. Kadivar handles straightforward extractions in-office. Gum Treatment

If gum infection or periodontitis is the source, a deep cleaning procedure known as scaling and root planing removes bacteria from below the gumline and allows the tissue to heal.
